On page 728, paragraph 1, line 4, please change "15 mL of calcium gluconate gel, diluted with 35 mL of 0.9% normal saline, was infused" so it now reads "10 ml of a 10% solution of calcium gluconate in 30-40 ml of normal saline solution is infused", and on line 6, please change reference "11" to "12".
On page 1848, under Management Considerations, paragraph 1, line 4, please change "naloxone, 0.4 to 2.0 mg" to "thiamine, 300 to 500mg" so it now reads "Patients with evidence of poor nutrition should receive thiamine, 300 to 500mg IV, before or early during administration of glucose to prevent Wernicke-Korsakoff syndrome".
On page 2008, Table 162.2, under the drug entry Pentobarbital, please change the text under usual dose "2 mg/kg initial, titrate in increments of 1 to 2 mg/kg" so it now reads USUAL DOSE "1 to 2 mg/kg initial, repeated every 3 to 5 minutes as necessary to desired effect or maximum dose". Please also change the text under maximum dose "6 or 200 mg/kg" so it now reads MAXIMUM DOSE "100 mg/dose". Please also change the text under onset "5 minutes" so it now reads ONSET "1 to 2 minutes".
On page 2017, Table 162.5, under the drug entry Fentanyl, under maximum dose please change "0.05 mg/kg/dose Usually 50 μg/dose" so it now reads MAXIMUM DOSE "1 to 2 μg/kg/dose Usually 50 μg/dose in adults".
On page 2049, Table 165.4, under Hyperosmolar Agents, please change "Mannitol 3%" and "Hypertonic saline (HTS)" so it now reads "Mannitol (20% solution)" and "Hypertonic saline (HTS) 3%".
